Definition
L-carnosine is a dipeptide of beta-alanine and L-histidine, present at high concentrations in skeletal muscle and brain. It is a genuine peptide by composition and it behaves in commerce like a nutrient, which is the source of nearly every confusion about it.
Muscle carnosine is raised most reliably not by taking carnosine but by taking beta-alanine, its rate-limiting precursor, which is why a large part of the literature that carries this molecule's name is a beta-alanine literature. This index tracks the substance because it holds a regulatory position no other molecule here does.
Evidence
Across three name variants PubMed returns 3,602 indexed records, of which 1,384 carry the Humans subject heading, 196 the clinical-trial publication type, and 158 the randomized-controlled-trial type [1]. This is by a wide margin the largest randomized literature of any unapproved substance in this index. It is above the itemization ceiling and is reported as a magnitude; the twenty-five most relevant randomized records were read to characterize it [1]. The publication-type filter was proved live in the same run against a control term returning 8,906 records under the clinical-trial type and 6,115 under the randomized type, the invalid tag returned zero for that control term, and a deliberately invalid record identifier errored in the same run [1].
Almost all of it is oral supplementation, and the results are mixed in the ordinary way of nutrition trials. Carnosine supplementation improved glucose control in adults with pre-diabetes and type 2 diabetes in one randomized controlled trial [2]; a fourteen-week randomized controlled trial in a comparable population reported that carnosine did not affect vascular and metabolic outcomes [3]. In cognition, supplementation improved outcomes in younger participants of one trial [4]. An early double-blind placebo-controlled study tested supplementation in children with autistic spectrum disorders [5]. Beyond those, the randomized set runs through sports performance, oral mucositis, xerostomia, depression and schizophrenia adjuncts — a supplement literature, spread thin across many indications.
What the set does not contain is a trial of an injected peptide. The distinction matters because the compounded and research-use-only market for this molecule is an injectable one, and a reader who checks the literature will find hundreds of randomized records describing something they are not being sold. A large count and a relevant count are different numbers, and here they diverge almost completely.
Lawful access
The lawful route is not the one this index usually describes. L-carnosine is sold openly as an oral dietary supplement, which is a distinct regulatory regime: a supplement does not require pre-market approval for safety and effectiveness, and its manufacturer may not make a claim to treat a disease. That is a lower evidentiary threshold than either an approved drug or a compounded preparation, and it is the regime almost every purchaser of this molecule is actually in.
As a bulk drug substance for compounding, the position is different. The section 503A bulks list FDA published May 14, 2026 lists L-Carnosine in Category 3 — the file for substances nominated without adequate support to evaluate — and in neither of the other two categories [6]. A pharmacy therefore has no route to compound it, whatever the supplement aisle contains.
Drugs@FDA holds no approved product with carnosine as an active ingredient; the same query form in the same session returned one application for bremelanotide and fourteen for liraglutide, and returned nothing for a deliberately invented ingredient name [7]. The substance was not among the seven peptides the advisory committee reviewed in July 2026 [6].
The practical consequence is worth stating plainly, because it is the question a reader arrives with. An oral supplement of this dipeptide is lawful to buy. An injectable preparation of it is not a supplement, has no approved product behind it, and has no lawful compounding route; the research-only page explains what is and is not true of material sold on that footing.
